4.  The Cook County Medical Examiner's Office is now requiring e-mail addresses for all funeral homes with a fax cremation account.

2017 Trade Show Passport

Don't forget to check out the Trade Show Passport, sponsored by American Wilbert,  in your registration packet!  Visit the exhibitor booths listed on the passport and have them stamp or initial their logo.  For each row/column/diagonal you complete, go to the FDSA Registration Desk and receive a raffle ticket.  Winners will be announced at the Cocktail Reception at 6:00 PM (you must be present to win).  This year's prizes include a $350 cash prize provided by the Funeral Service Credit Union, silver coins from Mid-States Recycling & Refining, a floral arrangement from Phillip's Flowers and a floral arrangement from Ashland Addison Florist Co.
